Summary
In reference to the management action plan to develop a memorandum of introduction to Native American tribes, this root cause analysis states that personnel were working under the impression that coordination was only required if the proposed project had a possible adverse effect on traditional religious or was of cultural importance. They overlooked the requirement to, “…meet periodically with each Indian Tribe”.
